Patient forms

Forms and notices.

Complete forms before your visit so your time goes to your care, not paperwork. Each one opens in a new tab, ready to print.

New patients

Before your first visit.

One packet covers registration, medical history, consent, telehealth consent, Care Between Visits consent, a records release, and our nondiscrimination notice. The consent includes our use of medical technology, including AI; here's how we use it.

Current patients

Consents to keep current.

Care Between Visits explains, and asks your consent for, care between appointments that may be billed to your insurance, such as Dr. Hayton answering a portal message. It lasts 12 months, and we’ll ask you to renew it each year. If you became a patient before October 5, 2026, we’ll also ask you to sign Technology in Your Care once.

Medicare patients

Coverage and wellness visits.

Complete the Medicare Coverage Questionnaire at your first visit and whenever your coverage changes.

For a wellness visit, use the Initial questionnaire if you’ve never had a Medicare Annual Wellness Visit anywhere. Otherwise, use the Annual one. Not sure? Use the Annual one, or call us.

Records & privacy

Your records and your rights.

To send your records to another doctor or person, use the Authorization to Release Health Information. To get a copy of your own records, ask for a correction, or use your other privacy rights, use Request About My Health Information. Or call, and we’ll help you complete either one.

Notices

Privacy, rights, and costs.

How we protect your health information, your civil rights and free language help, and your right to a cost estimate if you pay for care yourself.

Returning forms. Completed forms contain health information, so please bring them to your visit or fax them to (208) 684-2455 rather than emailing them. Need a form in large print, another language, or help filling it out? Call (208) 247-4279.
