Becoming a patient takes three steps.
An hour is reserved for your first appointment, and you'll leave with a plan for what comes next.
What happens
Book or call
Tell us who your previous provider was and we'll send a release form — we handle the records request from there.
Fill out the packet
Download it below, or arrive 10–15 minutes early and complete it here.
Your first visit
Use what you need of the hour, and leave with a plan for next steps. You’ll also get an invitation to the patient portal, where you can message us, see results, and request refills.
Forms
Complete these in advance so your visit isn't spent on paperwork. The packet covers registration, medical history, consent, telehealth consent, and a records release — all in one document.
- New Patient Packet
- Financial Policy & Patient Agreement
- Notice of Privacy Practices
- Your Right to a Good Faith Estimate
- Notice of Nondiscrimination
What to bring
- Photo ID
- Insurance card, plus supplemental card if you have one
- Your actual pill bottles, if you can
- Over-the-counter medications, vitamins, and supplements
- Names and phone numbers of any specialists you see
- Recent test results, if you have them
- A written list of your questions
Bring the bottles, not just a list. Dosages, prescribers, and fill dates on the labels catch discrepancies and interactions that a written list sometimes misses.
Insurance and cost
We're contracted with Traditional Medicare, several Medicare Advantage plans, Idaho Medicaid, and most major Idaho commercial plans. Molina takes effect September 15, 2026 and Humana on November 1, 2026 — call us if either is your coverage and you want to be seen sooner.
There is no missed-appointment fee. We just ask that you call if you need to cancel or reschedule.

Self-pay
Published rates and no surprise billing, with a courtesy discount when you pay in full at the time of service. Call and we'll walk through the fee schedule before you commit to anything.
Good Faith Estimate. If you're uninsured — or insured but not using it for a service — federal law gives you the right to a written estimate of expected charges. We provide one when you schedule and any time you ask; a question about cost counts as a request. If your final bill is $400 or more above the estimate, you may dispute it through the federal patient-provider dispute resolution process.
Things people ask before booking.
If yours isn't here, call (208) 247-4279. We'd rather answer it now than have you guess.
- Can I switch from my current primary care doctor?
- Yes. Book the first visit and sign the records release in the new-patient packet — we request your chart from your previous office so it's here before you arrive.
- How do I request a refill?
- Ask your pharmacy to send the request — that's the fastest route — or call us at (208) 247-4279. Allow two business days. Some medications require a visit before they can be renewed.
- Do I need a referral to see you?
- No. Some Medicare Advantage and commercial plans ask you to name a primary care physician — call us and we'll walk you through assigning Dr. Hayton.
- What happens if I'm admitted to the hospital?
- The hospital's physicians manage your care while you're admitted. We get the discharge summary and see you within two weeks to pick things back up and reconcile any medication changes.
- What if I need to be seen on a Friday?
- The office is open Monday through Thursday. For urgent problems outside those hours, call (208) 247-4279 and follow the prompts. For a medical emergency, call 911.
Book a visit, or call and ask us anything.
New patients are welcome. We confirm your coverage and give you a cost estimate before you're seen — so there are no surprises.
