Privacy Policy

What we collect

This website uses Google Analytics to understand how the site is used — which pages are visited, roughly where visitors come from, and which links are clicked, such as the booking, phone, and form links. Google Analytics uses cookies to do this. We use the results only as aggregate reports; they are not linked to your name and are never combined with any medical record.

Our web host also records standard server logs as part of delivering the site.

Advertising

We advertise on Google. When you reach this site from one of our Google ads, the link carries a reference number from Google, and our Google tag stores it in a cookie on your device. This lets Google Ads report how many visits from our ads led to a phone call, a click to book, or a contact-form message, so we can tell whether the ads are working.

Google receives information about these visits under its own privacy terms, described in how Google uses information from sites that use its services (opens in new tab). We do not send Google your name, your message, or anything about your health, and we do not use this site to show you ads later or to build advertising audiences.

To limit how Google uses information for ads, visit My Ad Center (opens in new tab), or block or clear cookies in your browser. The site works without them.

The contact form

The form on our Contact page collects your name, phone number, an email address if you give one, the reason you selected, how and when you would like to be reached, how you heard about us, and anything you type in the message box. We ask for this only so a person can call you back.

Submissions are processed and stored by Netlify, the company that hosts this website, and are emailed to the practice. Netlify is not acting as a HIPAA business associate for this form, which is why the form asks you to keep medical details out of it — and why the form is not a way to reach us about a medical problem. We keep submissions only as long as we need them to answer you and to open a chart if you become a patient, and we delete them from the host after that.

We do not sell this information, use it for advertising, or share it with anyone outside the practice and the host described above. If you would like your submission deleted, write to info@haytonprimarycare.com and we will remove it.

Maps

The maps on our Home and Contact pages do not load until you click to show them. Once you do, Google may set cookies and receive your IP address under its own privacy terms. If you would rather not, the full address is written out as text on every page.

Please don't send health information

Neither email nor the contact form is a secure channel, and neither is intended for medical information. Please do not include symptoms, conditions, or medications in either one. We collect medical details securely by phone before your first visit, and through the patient portal once you are established.

Third-party services

Scheduling, payments, and telehealth are handled by third-party vendors under business associate agreements where required. Those services have their own privacy terms, which apply when you use them.
