Privacy Policy

This policy covers the haytonprimarycare.com website only. It does not govern the collection or use of your protected health information as a patient. Your health information is governed separately by our Notice of Privacy Practices, as required by HIPAA.
What we collect
This website uses Google Analytics to record anonymous, aggregate information about how the site is used — which pages are visited, roughly where visitors come from, and which links are clicked. This helps us understand whether the site is useful. It is not linked to your identity and is never combined with any medical record.
Our web host also records standard server logs as part of delivering the site.
The contact form
The form on our Contact page collects your name, phone number, an email address if you give one, the reason you selected, how and when you would like to be reached, how you heard about us, and anything you type in the message box. We ask for this only so a person can call you back.
Submissions are processed and stored by Netlify, the company that hosts this website, and are emailed to the practice. Netlify is not acting as a HIPAA business associate for this form, which is why the form asks you to keep medical details out of it — and why the form is not a way to reach us about a medical problem. We keep submissions only as long as we need them to answer you and to open a chart if you become a patient, and we delete them from the host after that.
We do not sell this information, use it for advertising, or share it with anyone outside the practice and the host described above. If you would like your submission deleted, write to info@haytonprimarycare.com and we will remove it.
Maps
The map on our Contact page does not load until you click to show it. Once you do, Google may set cookies and receive your IP address under its own privacy terms. If you would rather not, the full address is written out as text on every page.
Please don't send health information
Neither email nor the contact form is a secure channel, and neither is intended for medical information. Please do not include symptoms, conditions, or medications in either one. We collect medical details securely by phone before your first visit, and through the patient portal once you are established.
Third-party services
Scheduling, payments, and telehealth are handled by third-party vendors under business associate agreements where required. Those services have their own privacy terms, which apply when you use them.
