Dr. Derek Hayton, DO — Hayton Primary Care, Boise, Idaho
DO — Doctor of Osteopathic Medicine Board Certified · ABFM & ABPM Fellowship Trained · Addiction Medicine Idaho since 2014

Dr. Derek Hayton, DO

Board-Certified · Family Medicine & Addiction Medicine

Dr. Hayton is a board-certified, fellowship-trained physician focused on adults managing multiple chronic conditions, multiple specialists, and medications that no one has taken the time to sort out.

“Good medical care requires time, continuity, and accountability. Patients deserve a physician who understands their full picture, explains decisions clearly, and takes responsibility for coordinating care across a complex healthcare system.”

After years in hospital medicine, large primary care groups, and the VA, a pattern became clear: patients with complex conditions were left coordinating their own care across specialists, medications, and systems — with no single physician responsible for the whole picture. Hayton Primary Care was built to offer a different model.

That model is simple: one physician, enough time, and a practice structure designed around longitudinal care — not throughput.

Many patients come here after feeling rushed or disconnected.

This practice was designed for adults who need more than a 15-minute appointment and a different provider every visit. If any of these sound familiar, you’re exactly who we built this for.

  • Managing multiple conditions and want one physician overseeing everything
  • Taking several medications and want careful, coordinated review
  • Seeing multiple specialists who are not communicating with each other
  • Want more time during visits and better follow-through between them
  • Frustrated by not being able to reach your physician directly
  • Want a physician who will actually know your history

Aug 24, 2026

Hayton Primary Care — Boise

Complex adult primary care focused on patients managing multiple chronic conditions. Opening August 24, 2026.

Better care comes from knowing the patient well — and following their case over time.

The structure of this practice reflects that belief. Three things are different here from what most patients have experienced.

You work directly with your physician

You meet with Dr. Hayton at every visit. Your care plan and decisions are handled by the physician who knows your case — not a surrogate, call center, or rotating staff.

One-hour new patient appointments

Your first visit is scheduled for a full hour. That’s enough time to actually review your history, medications, and concerns — not just the top two on the list.

Care continues between visits

For eligible patients, ongoing coordination — medication review, specialist communication, and follow-through — continues between appointments. Your care doesn’t pause until the next scheduled visit.
