Medicare changed that. Chronic Care Management funds the between-visit coordination most practices can’t afford: monthly check-ins, a written care plan, and specialist communication. This practice was built around it.

For eligible Medicare patients, this is a covered service. Depending on your supplemental coverage, many patients pay little or nothing out of pocket — we confirm your specific cost before enrollment.
This is your primary care practice. You establish care with Dr. Hayton as your physician. CCM is a component of care available to eligible Medicare patients — not a separate program enrolled in elsewhere.

Most Medicare patients with ongoing conditions qualify
CCM requires two or more chronic conditions expected to last at least 12 months. If you’re managing a handful of health issues and feel like no one has the full picture — you’re exactly who this is for.

What Medicare asks for
- Enrolled in Original Medicare Part B or Medicare Advantage
- Two or more chronic conditions lasting 12+ months
- Conditions placing you at risk of decline or exacerbation
- Written consent to enroll (completed at your first visit)
- One designated CCM physician
Only one provider can bill CCM per patient per month. If you’re currently enrolled elsewhere, we’ll walk you through how to transition.

What CCM costs — and what Medicare pays
CCM is a covered Medicare benefit. For most patients with supplemental insurance, out-of-pocket costs are minimal or zero. We verify your specific coverage before enrollment so you know what to expect.
Medicare Part B
Medicare covers 80% of the approved CCM amount under Part B. The remaining 20% is typically covered by your Medigap supplement — leaving most patients with little or no out-of-pocket cost.
Advantage Plans
Most Medicare Advantage plans cover CCM, though cost-sharing varies by plan. We verify your plan before enrollment and tell you exactly what to expect before you commit to anything.
Supplemental Insurance
Patients with Medigap typically pay nothing for CCM. Their supplement covers the 20% that Medicare doesn’t — making this the most common zero-cost scenario for our Medicare patients.
